Wellspring Home Health Center offers a wide variety of home medical and support care services to meet the unique needs of each individual patient. All of our programs are coordinated by highly trained, experienced and licensed home care nurses whose focus is providing the most appropriate, professional and compassionate care for our patients in a home care setting.

We approach all patients as individuals, carefully assessing all medical conditions and needs before assigning a caregiver. Upon completion, we develop an individualized plan of care and assign a professional that best matches the needs of the client.

Skilled Nursing Services


Certified Nursing/Home Health Aides

Our compassionate, trustworthy and certified Home Health Aides are available to assist with personal care and hygiene, Activities of Daily Living (ADL), assist with ambulation’s, grooming, medication reminders, feeding assistant and Meal preparation/planning and give emotional and psychological support to clients and their family. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with the client’s nurses, physician, and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our Certified Home Health Aides team is here to give you peace of mind, knowing that your loved one is in good hands.


Skilled Nursing Care

Wellspring Home Health Center provides highly skilled and compassionate nurses on a full time, part-time or per visit basis, who work closely with the client, their physician, family members and provide services under Physicians order of care to:  maintain the client’s health and prevent further illness and injury, wound care, chronic disease management such as COPD, Hypertension, CHF, Diabetes etc., pain management, Ostomy care, Post-surgical and post stroke care, Fall prevention, anticoagulant management, catheter care, wound care etc.


Physical Therapy

At Wellspring Home Health Center, our Physical Therapists will develop a plan according to the Physician order using treatment techniques to promote the ability to move, reduce pain, restore function and prevent disability to our clients, build rapport and utilizing effective communications skills with patient and staff, provide an exceptional level of customer service while working with patients, execute daily patient treatments and complete necessary and appropriate documentation in a timely manner.

Individuals who have a disability or an injury may require rehabilitative guidance to regain strength, mobility, and physical function through services such as:

  • Mobility Assistance
  • Pain Management
  • Exercise Programs
  • Massage Therapy
  • Movement and Joint Motion Analysis
  • Coordination and Balance Testing


Occupational Therapy

Provides skilled occupational therapy services on intermittent basis to patient in their home in accordance with the physician’s orders. Our Occupational Therapists will guide and instructs clients in therapeutic self-care such as the use of wheelchairs, braces, splints and prosthetic and orthotic devices, teach and educate client and family in the total occupational therapy program in relearning new skills in living and working environment and establishes goal based on the patient’s current level of functioning and potential improvement and perform re-evaluations as needed or indicated.

Speech Therapy

Wellspring Home Health Center provides skilled speech pathology service on an intermittent basis to patients in their homes in accordance with the physician’s orders.  Our Speech-Language Therapists help patients develop their language and speech skills to regain control and increase their communication.

  • Mild to severe Autism
  • Cleft Palate
  • Developmental Delays
  • Cerebral Palsy
  • Traumatic brain injury

  • Cognitive communication disorders

Medical Social Worker

At Wellspring Home Health Center, our Medical Social Worker provides medical social services to patients in their homes in accordance with physician orders and under the supervision of the Clinical Director. They will assist patient and their families’ members cope with personal, physical and emotional effect of their disease or disability. They will also assist with financial resources and community assistance such as home meal delivery or support groups.

Personal Care Services

Wellspring Personal Care Services provide support to individual activities of daily living (ADL’s) such as bathing, dressing, eating, toileting, and eating as well as instrumental activities of daily living (IADL’s) such as shopping, running errand, laundry, and light house keeping, which makes living at home much easier for our clients and their families. Our mission is to provide professional and high quality services to clients in their homes by assisting them achieve the highest level of potential in their day-do-day self-care activities. We are committed to providing high quality care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional’s point of view.
